Qualifications

• Experience in care coordination or case management in health care settings.
• Familiarity with Medicaid/Medicare or social drivers of health (SDoH).
• Strong communication and documentation skills.

Responsibilities

• Serve as the primary care manager for a panel of members with chronic and complex health care needs.
• Perform comprehensive needs assessments and develop person-centered care plans.
• Collaborate with care team, providers, and community resources to implement plans.
• Assist members with HCBS supports and services; provide referrals as needed.
• Coordinate care transitions after ER visits or hospital stays; support discharge planning.
• Educate and coach members on self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are represented by the health care team.
