UnitedHealthcare is hiring a Care Coordinator to manage a panel of members with chronic and complex needs. This role supports medical, behavioral, and socioeconomic needs to promote appropriate utilization and better quality of care.

You will work in a fast-paced environment focusing on person-centered care plans, collaboration with internal teams, and community resources, with remote work options for the Idaho region.

Qualifications

• Idaho license in one of listed clinical disciplines (LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT, or LPN/LVN).
• 1+ year experience with Medicaid/Medicare or SDoH needs.
• 1+ year MS Office (Word, Excel, Outlook) experience.
• Must have a quiet home workspace with PHI security and reliable internet; must have ability to travel up to 50%.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs or wellness assessments with medical, behavioral, functional, cultural, and social drivers of health.
• Collaborate with care team, providers, and community resources to implement care plans and remove barriers.
• Assist members with HCBS support and services.
• Provide referrals to internal or community-based resources as appropriate.
• Support discharge planning and coordination after ER, inpatient, or SNF stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate to ensure members' needs and choices are represented by the care team.
• Assist providers and facilities with nonclinical questions and connect to Health Plan resources.

Education

2-year degree (or higher) AND 2+ years of healthcare experience
