Job summary

The candidate will coordinate with providers, assist with HCBS services, and support transitions from ER or SNF settings while maintaining HIPAA compliance. Remote flexibility is available for qualified individuals.

Qualifications

• Current and unrestricted Idaho license in one of the listed professions.
• 1+ years of experience with Medicaid/Medicare or significant SDoH needs.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Drivers license and ability to travel within territory up to 50% of time.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs assessments and develop person-centered care plans.
• Collaborate with internal care teams and community resources to implement plans.
