Job summary

UnitedHealth Group is seeking a Care Coordinator to manage a panel ofdual-eligible members with chronic conditions, delivering person-centered care across medical, behavioral, and social needs. The role supports a broader team with clinical and non-clinical duties to improve care quality, access, and outcomes.

This remote-friendly position requires strong organizational skills, ability to multitask, and a commitment to equitable care across populations.

Qualifications

• Current and unrestricted Idaho license in one of the listed clinical licenses or equivalent (LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN).
• 1+ years of Medicaid/Medicare or SDoH-focused experience.
• 1+ years of MS Office (Word, Excel, Outlook).
• Driver’s license and ability to travel up to 50% of the time.
• Home workspace compliant with PHI protections and reliable internet.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs or wellness assessments with clients.
• Collaborate with care team and community resources to implement plans.
• Assist members in obtaining HCBS supports and services.
• Provide referrals to internal and external resources as appropriate.
• Support discharge planning and care transitions from ER or SNF.
• Educate and coach members on self-management and healthy lifestyle changes.
• Advocate for patient needs within the care team.
• Support nonclinical questions and connect providers to Health Plan resources.
