Job summary

UnitedHealthcare is seeking a Care Coordinator to serve as the primary care manager for a panel of dual eligible members. You will engage with members to conduct comprehensive assessments, develop individualized care plans, and coordinate with providers and community resources to improve outcomes.

The role emphasizes protecting PHI, working in a fast-paced environment, and leveraging internal teams to address social determinants of health. Remote work options are available within the Idaho area.

Qualifications

• Experience with Medicaid/Medicare and social determinants of health (SDoH)
• Proficiency in MS Word, Excel, Outlook
• LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT or LPN/LVN licensure or higher

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Engage with members to complete needs assessments and develop person-centered care plans.
• Collaborate with internal care team and community resources to implement plans and remove barriers to care.
• Assist members with obtaining HCBS supports and services.
• Provide referrals and linkage to appropriate providers and resources.
• Support discharge planning and coordinate care transitions after ER or facility stays.
• Educate and coach members on self-management and healthy lifestyle changes.
• Advocate for members to ensure needs are represented by the care team.

2-year degree + 2+ years healthcare experience
