Job summary

UnitedHealth Group seeks a Care Coordinator in Idaho to manage a panel of members with complex health needs. You will perform comprehensive assessments, create person‑centered care plans, and coordinate with clinicians and community resources to improve care quality.

The role emphasizes collaboration, PHI protection, and travel within the assigned territory. You will support transitions, provide education, and advocate for optimal health outcomes in a fast paced setting.

Qualifications

• Current Idaho license as LCSW/LMSW/LSW/LCPC/LPC/LMFT or LAMFT.
• 1+ years of Medicaid/Medicare or SDoH experience.
• Proficient with MS Office suite and PHI handling.
• Must have reliable home workspace and licensed driver with ability to travel up to 50%.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments and develop person‑centered care plans.
• Collaborate with internal team, providers, and community resources to implement plans.
• Assist with HCBS supports and referrals to appropriate services.
• Support discharge planning and care transitions from ER or facility.
• Provide education to promote self‑management and healthy lifestyle changes.
• Advocate for member needs within the health care team.
• Address nonclinical questions and connect providers to the right Health Plan resources.
