UnitedHealth Group is seeking a Care Coordinator in Idaho to manage a panel of dual-eligible members with complex health needs. You will perform comprehensive assessments, create person-centered care plans, and collaborate with providers and community resources to improve outcomes.

This role supports remote work with flexible scheduling and requires licensure in Idaho, plus experience with Medicaid/Medicare and SDoH challenges. The team emphasizes multidisciplinary care and patient advocacy.

Qualifications

• Current LCSW, LMSW, LSW, LCPC, LPC, LMFT, or LAMFT in Idaho.
• 1+ year experience with Medicaid/Medicare or social drivers of health (SDoH).
• 1+ year experience with MS Office (Word, Excel, Outlook).
• Driver's license, reliable transportation, and ability to travel up to 50% for member meetings.
• Home workspace with ability to secure PHI and reside where a UnitedHealth Group approved internet connects.

Responsabilités

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs or wellness assessments addressing medical, behavioral, functional, cultural, and social drivers of health.
• Collaborate with care team, providers, and community resources to implement care plans and remove barriers.
• Assist members in obtaining HCBS supports and services.
• Provide referrals to internal teams or community providers as appropriate.
• Support discharge planning and care transitions after ER visits or SNF stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for members to ensure needs and choices are represented by the care team.
