Qualifications

• Must hold an Idaho license in one of the following: LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT or LPN/LVN, or have a 2-year degree and 2+ years healthcare experience.
• 1+ years of experience working with Medicaid/Medicare or with significant social drivers of health (SDoH).
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to a quiet workspace at home and ability to secure PHI.
• Reside in an area with reliable high-speed internet.
• Ability to travel within the assigned territory up to 50% of the time.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Engage members to complete comprehensive needs or wellness assessments and develop person-centered care plans.
• Collaborate with internal care teams, providers, and community resources to implement plans and overcome barriers.
• Assist members in obtaining HCBS supports and services.
• Provide referrals to appropriate internal or community resources.
• Support discharge planning and care transition after ER visits or inpatient stays.
• Educate and coach members in self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are represented by the care team.
• Provide nonclinical support to providers and facilities and connect them to appropriate Health Plan resources.
