Qualifications

• Experience in care coordination for Medicaid/Medicare or social determinants of health (SDoH).
• Proficient in MS Office applications (Word, Excel, Outlook).
• Licensed in Idaho or equivalent LCSW/LMSW/LSW/LCPC/LPC/LMFT or LPN/LVN in good standing.
