Job summary

UnitedHealth Group in Meridian, ID is hiring a Field Care Coordinator to manage a panel of members with chronic and complex health needs. The role supports medical, behavioral, and social drivers of health through person-centered care plans, assessments, and care coordination with the internal team and community resources.

This remote-friendly, full-time role requires strong organization, clinical and non-clinical coordination, and the ability to travel up to 50% in the territory.

Qualifications

• Idaho license in a qualifying field (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT or LPN/LVN) is required.
• At least 1 year of Medicaid/Medicare or SDoH-focused experience.
• Proficiency with MS Office tools (Word, Excel, Outlook).
• Able to travel up to 50% within the assigned territory.

Responsibilities

• Serve as primary care manager for dual-eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with care teams, providers, and community resources to implement plans.
• Assist with HCBS referrals and care coordination across settings.
• Support discharge planning and transitions after ER visits or facility stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for member needs to ensure alignment with care teams.
• Answer nonclinical questions and connect providers to Health Plan resources.
