Job summary

Optum in Meridian, Idaho seeks a Care Coordinator to manage a panel of members with complex health care needs. You will perform comprehensive assessments, develop person-centered plans, and coordinate with internal teams and community resources to improve care outcomes.

Excellent organizational skills and multitasking ability are essential. Remote work is available with an expectation of on-site collaboration as needed.

Qualifications

• Current Idaho license in eligible fields (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT, or LPN/LVN).
• 2-year degree (or higher) with healthcare experience; Medicaid/Medicare exposure preferred.
• Experience with MS Office (Word/Excel/Outlook) required.
• Ability to travel within territory up to 50% as needed.
• Secure workspace at home and access to reliable high-speed internet.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs assessments and develop person-centered care plans.
• Collaborate with care teams and community resources to implement plans.
• Assist with obtaining HCBS supports and services; provide referrals.
• Coordinate care transitions after ER visits or facility stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for member needs within the health care team; ensure PHI protection.
• Assist nonclinical questions and connect providers to Health Plan resources.
