UnitedHealth Group is hosting a care coordinator role focusing on primary management of dual eligible members. The position supports comprehensive assessments, develops person‑centered care plans, and partners with providers and community resources to improve health and quality of life.

Remote work is available with Boise area flexibility; travel up to 50% may be required to meet members and providers, and a strong PHI security posture is expected.

Qualifications

• Current LCSW, LMSW, LSW, LCPC, LPC, LMFT, or LAMFT in Idaho.
• 1+ years of experience working with Medicaid/Medicare or with significant social drivers of health (SDoH).
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Driver's license and ability to travel within the territory up to 50%.
• Access to a designated quiet workspace at home with the ability to secure PHI.
• Reside in a location with access to high‑speed internet or a suitable connection in Idaho.

Responsabilités

• Serve as the primary care manager for dual eligible members.
• Engage members to complete comprehensive needs and wellness assessments, including medical, behavioral, functional, cultural, and social drivers of health.
• Collaborate with internal care team, providers, and community resources to implement care plans and remove obstacles.
• Assist members with obtaining HCBS supports and services.
• Provide referrals to internal or community resources as appropriate.
• Support discharge planning and coordinate care transitions after ER visits or inpatient stays.
• Educate and coach members to promote self‑management of care needs and healthy lifestyles.
• Advocate for members to ensure their needs are represented by the health care team.
• Assist with nonclinical questions and connect to Health Plan/UnitedHealth Group resources.
