Job summary

UnitedHealthcare is seeking a Care Coordinator to serve as the primary care manager for a panel of dual eligible members. You will conduct comprehensive assessments, develop individualized care plans, and coordinate with the care team to address medical, behavioral, and social drivers of health.

The role requires strong organizational skills, the ability to multitask, and the flexibility to work remotely up to 50% in Lewiston, ID or surrounding areas, with occasional in-person visits as needed.

Qualifications

• Idaho license in LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN or 2-year degree with 2+ years healthcare experience.
• 1+ years of experience with Medicaid/Medicare or SDoH needs.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to a quiet workspace at home to secure PHI.
• Reside in an area with access to UnitedHealth Group approved high-speed internet.
• Ability to travel within the assigned territory up to 50% of time.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments and develop person-centered care plans.
• Collaborate with internal care teams, providers, and community resources to implement care plans and remove barriers.
• Assist members with obtaining HCBS supports and services.
• Provide referrals and liaise with internal or community-based providers as needed.
• Support discharge planning and care transitions after ER visits, inpatient stays, or SNFs.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are met by the health care team.
• Support nonclinical questions from providers and facilities, connecting to the correct Health Plan or UHC resources.
