Job summary

SCAN Health Insurance seeks a Care Coordinator to enhance the well-being of SCAN members by assessing, planning, coordinating, and monitoring holistic needs. You will address care coordination needs, process referrals, and help members navigate the healthcare system.

Qualifications

• High School diploma required; some college preferred.
• 0-3 years working in healthcare or customer service.
• Ability to use motivational interviewing techniques to understand member concerns.
• Ability to work in an environment with ongoing coaching and feedback.
• Strong written and verbal communication skills; basic interpersonal skills.
• Ability to collaborate effectively within a multidisciplinary team.
• Ability to maintain confidentiality; knowledge of CMS and DHCS regulations.
• General knowledge of medical terminology and local community resources for seniors.
• Spanish strongly preferred (conversational, reading, writing).
• Microsoft Office skills (Word, Excel, PowerPoint, Outlook, Teams) required.

Responsibilities

• Perform assessments and screenings to determine appropriate interventions and referrals.
• Act as triage point of contact with members, providers, and medical groups for benefits and to troubleshoot access issues.
• Use motivational interviewing to understand member goals and concerns.
• Coordinate care utilization of member benefits and community resources to resolve access to care issues.
• Complete timely documentation (assessments, care plans, case notes) in multiple systems.
• Demonstrate critical thinking to resolve issues with members, providers, and vendors.
• Collaborate with county offices and governmental agencies as needed.
• Exhibit professionalism and empathy in all interactions with members and provider partners.
• Follow department procedures and training materials for workflows and documentation.
