Job summary

SCAN is seeking a Care Coordinator - SCAN Temp to support members in aging in place by coordinating holistic needs, referrals, and services. You will assess, plan, coordinate, and monitor care, working to resolve access issues and optimize health outcomes.

Responsibilities include conducting assessments, triaging inquiries, and documenting care plans across systems. We value empathy, professionalism, and teamwork in a mostly remote setting.

Qualifications

• High School Diploma required; Some college preferred.
• 0-3 years in healthcare or customer service.
• Spanish strongly preferred: Conversational, Reading, Writing.
• Proficiency in Microsoft Office and basic computer skills.

Responsibilities

• Perform assessments and screenings to determine interventions and referrals.
• Triage and contact members, providers, and medical groups for benefits/programs.
• Address member goals and concerns using motivational interviewing.
