Job summary

SCAN seeks a Remote Part-Time RN to enhance care for seniors via telephonic care coordination. You will assess, educate, and coach members enrolled in LTSS/Medicare programs, developing individualized plans and coordinating with care teams to prevent hospital visits and deterioration.

You will work within regulatory standards (CMS/DHCS) and deliver documentation across systems while supporting SCAN's mission to keep older adults healthy and independent.

Qualifications

• ADN/ASN required; RN licensure in CA.
• BSN strongly preferred; advanced degree encouraged.
• 3+ years managed care, healthcare, or case management experience.
• 2+ years remote work experience preferred; ability to work telephonically.

Responsibilities

• Perform initial and annual telephonic assessments for eligible members.
• Ensure clinical appropriateness of care plans using guidelines and best practices.
• Participate in interdisciplinary planning and case conferences as needed.
• Provide education, coaching, and disease management for chronic conditions.
• Promote member engagement and self-management outcomes.
• Document care plans, service plans, authorizations, and progress notes in multiple systems.
