SCAN Group is seeking a Patient Care Coordinator to support the SCAN Mobile Care Team (Embrace) and coordinate care for members, promoting independence and health management. The role involves clinical support, appointment scheduling, and handling patient information with strict privacy practices.

Qualifications

• Associate's Degree or equivalent experience.
• 2+ years of related experience as a medical assistant, medical office administrator, or care coordinator.
• Experience with Medicare Advantage or long term care communities preferred.
• Ability to communicate effectively verbally and in writing.
• Cultural competence and sensitivity; multilingual skills preferred.
• Knowledge of HIPAA requirements.

Responsibilities

• Work closely with SCAN Mobile Care Team clinicians to deliver an excellent healthcare experience to members.
• Support SMCT by performing administrative responsibilities with minimal supervision including: refill prescriptions, communicate with vendors, order equipment, process lab orders, and fill patient forms.
• Schedule specialty appointments and coordinate transportation.
• Answer calls professionally and route appropriately.
• Maintain inventory of supplies and stock and order as necessary.
• Collect and organize patient medical information and reports while protecting privacy.
• Identify opportunities for improvements from patient feedback and propose solutions.
