Job summary

This fast-paced role offers remote work flexibility for residents near Coeur d'Alene, ID, with up to 50% travel. Idaho licensure or equivalent, Medicaid/Medicare experience, and proficiency with MS Office

Qualifications

• Current Idaho license in one of the listed social work or nursing professions or 2-year degree with 2+ years healthcare experience
• 1+ years working with Medicaid/Medicare or SDoH needs
• 1+ years of MS Office experience
• Access to a quiet home workspace and ability to protect PHI
• Reliable high-speed internet in location
• Must be able to travel up to 50% within territory

Responsibilities

• Serve as primary care manager for dual eligible members
• Conduct comprehensive needs or wellness assessments
• Develop and implement person-centered care plans
• Collaborate with internal care team and community resources
• Assist with HCBS support and referrals
• Coordinate care transitions after ER visit or SNF
• Provide education to support member self-management
• Advocate for members' needs within the care team
• Support nonclinical inquiries to connect with PHS and health plan resources
