Qualifications

• Idaho license in one of listed professions (RN, LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT).
• 1+ years with Medicaid/Medicare or SDoH needs.
• 1+ years with MS Office (Word, Excel, Outlook).
• Driver's license and travel up to 50% of time.
• Access to a quiet home workspace with PHI security.

Responsibilities

• Serve as primary care manager for dual-eligible members.
• Conduct needs assessments including medical, behavioral, and social drivers of health.
• Collaborate with care team to implement care plans.
• Assist members with HCBS supports and services.
• Provide referrals to internal and community resources.
• Support discharge planning and transitions.
• Educate and coach for self-management.
• Advocate for members and families.
• Address nonclinical questions with providers and health plans.
