Job summary

Acentra Health is seeking a Care Coordinator, Supervisor to lead a fully remote team supporting members through transitions of care and access to services. You will coach staff, manage workloads, and ensure high-quality documentation while guiding non-clinical care coordination efforts.

This role requires strong supervisory skills, knowledge of Medicaid/Medi-Cal programs, and experience in fast-paced call centers.

Qualifications

• Associate Degree or 6+ years of equivalent healthcare experience with 5+ years in relevant healthcare fields.
• 4+ years of care coordination, case management, social work or related healthcare experience.
• 2+ years of supervisory or team leadership in a healthcare or care coordination setting.
• Experience overseeing call center operations and monitoring service levels.
• Strong understanding of HIPAA and privacy requirements.

Responsibilities

• Provide day-to-day supervision and direction to Care Coordinators in a remote team.
• Assign workloads, monitor priorities, and ensure timely service delivery.
• Coach and develop staff; conduct performance reviews and address concerns.
• Monitor productivity, quality, documentation, referrals, and outcomes.
• Oversee member transitions of care, provider referrals, and initial appointments.
• Ensure accurate documentation in electronic systems and maintain privacy compliance.
• Identify barriers to access and collaborate with partners to resolve issues.
• Promote culturally responsive services and continuous improvement.
• Lead onboarding and training for new Care Coordinators.
