Job summary

Acentra Health is seeking a Senior Program Director to lead a new CA UIS Care Coordination program across California. You will provide strategic and operational leadership, oversee care coordination workflows, and partner with clinical, operational, and community organizations to ensure timely, equitable, and person-centered support.

The role is remote with California residence required; preferred candidates in the LA or Sacramento areas. Travel up to 25% may be required.

Qualifications

• Bachelor's Degree in healthcare, nursing, social work or related field.
• 8+ years' experience in case management and care coordination.
• 8+ years' experience in healthcare operations or CA public sector health programs, preferably in Medi-Cal or MCO environments
• 10+ years' experience in leadership/management roles, and engaging stakeholder relationships.

Responsibilities

• Direct program through objectives and contract compliance; monitor metrics and daily operations.
• Collaborate with medical directors and management teams to improve processes, determine training needs and improvement opportunities.
• Engage community partners and stakeholders in delivering member outreach.
• Provide leadership to staff across quality, Case Management, Care Coordination, provider training/education, data collection, analysis and reporting, and technical assistance to ensure compliance with accreditation standards.
• Oversee contract scope enhancement, new RFP cycles and related new business.
• Oversee budget review and analysis to ensure financial success of the program.
• Assess systems to ensure high customer satisfaction across stakeholders.
• Collaborate with management to create an organizational structure that enables effective processes and localized community engagement in the southern region.
• Assist in determining professional development opportunities for staff and teams.
• Read, understand, and adhere to all corporate policies including HIPAA Privacy and Security Rules.
