Job summary

Acentra Health is seeking a Senior Program Director to lead a new CA UIS Care Coordination Program. The role focuses on strategic and operational oversight, working with clinical, operational, provider and health plan partners to ensure equitable, person-centered care across California.

Remote work is available but candidates must reside in California. Ideal candidates have 10+ years in leadership, 8+ years in case management and healthcare operations, and a strong track record in stakeholder

Qualifications

• Bachelor's degree required in healthcare, nursing, social work or related field.
• 8+ years' experience in case management and care coordination.
• 8+ years' experience in healthcare operations or CA public sector health and human services programs, preferably Medi-Cal or MCO environments.
• 10+ years' experience in leadership/management roles and stakeholder relationships.
• Must reside in California and be able to work remotely.
• Flexibility to travel up to 25%.

Responsibilities

• Direct the program through objectives and contract compliance; monitor metrics and daily operations.
• Collaborate with program leaders and corporate teams to improve processes and training.
• Engage community partners and stakeholders in outreach to members.
• Provide leadership and supervision to staff including QA, care coordination, training, data collection, analysis, and reporting.
• Work with management to enhance contract scope and pursue new business opportunities.
• Oversee budget review and financial performance of the program.
• Assess systems and workflows to ensure high customer satisfaction for stakeholders.
• Develop organizational structure to support field-based outreach and regional coordination.
• Assist in professional development opportunities for staff and teams.
• Adhere to HIPAA policies and company privacy/security rules.
