Acentra Health is seeking a Senior Program Director to lead a new CA UIS Care Coordination program, guiding strategic and daily operations across California. You will partner with clinical, operational, provider, health plan, and community-based organizations to ensure timely, equitable, and person-centered care while managing workflows, transitions, and social determinants of health.

Qualifications

• 8+ years of case management and care coordination experience.
• 8+ years in healthcare operations or CA public sector health and human services programs.
• 10+ years in leadership/management roles with stakeholder engagement.
• Must reside in California; remote work possible.
• Up to 25% travel acceptable.

Responsibilities

• Direct program objectives and contract compliance; monitor metrics and daily operations.
• Collaborate with medical director, management teams, and corporate support to improve processes and training.
• Engage community partners and stakeholders for member outreach.
• Provide leadership and supervision for staff including quality, care coordination, and data reporting to ensure accreditation standards.
• Oversee contract scope, new RFP cycles, and new business opportunities.
• Oversee budget review and financial performance of the program.
• Assess systems for customer satisfaction and workflow improvements; ensure localized outreach in CA.
