Job summary

Talkspace is seeking an experienced Care Coordinator to join our Referral Management team and support patients throughout their care journey. This remote role focuses on monitoring engagement, identifying barriers, and coordinating with providers and facilities when escalation is needed.

• Ability to manage multiple patients and priorities in a fast-paced environment.

• Comfortable using multiple systems and platforms.

Responsibilities

• Monitor patient lead-up requirements and paperwork for first appointments.
• Follow up on outstanding patient requirements and barriers to activation.
• Ensure patient accounts are activated and prepared for care.
• Provide patients with clear information on next steps and care coordination processes.
• Respond to patient questions and ensure follow-through.
• Monitor patients during initial care journey and maintain engagement.
• Identify patients at risk of disengagement and take appropriate actions.
• Follow up on missed appointments and coordinate with internal teams.
• Prevent incorrect billing for missed first appointments and escalate as needed.
• Maintain timely documentation of outreach, engagement, and follow-up activities.
• Escalate complex scheduling or care-transition issues to the Care Coordinator Manager.
• Collaborate with providers and internal teams to support patient needs.
