Qualifications

• Minimum 1+ years in multicultural community setting.
• Knowledge of customer service principles.

Responsibilities

• Provides telephonic care coordination outreach to support preventative health activities.
• Delivers comprehensive care coordination services to enrollees.
• Conducts referrals to Personal Health Consultants and primary health plans as needed.
• Provides real-time resources and transitions members to Personal Health Consultants.
• Upholds health equity with patient-centered, respectful engagement.
• Maintains metrics for telephonic outreach and member engagement.
• Collaborates with supervisor/manager to meet contract requirements and report work hours accurately.
• Adheres to HIPAA and Privacy and Security Rules.
