Job summary

Duly Health and Care is seeking a Specialty Care Coordinator to conduct telephonic outreach with high-risk patients, coordinate appointments, and connect them with resources. You will maintain proactive communication with physicians and care partners to ensure timely escalation and alignment with treatment plans.

The role emphasizes independent work in a remote environment, strong communication, organization, and empathy.

Qualifications

• Active Medical Assistant certification or equivalent clinical credential.
• Familiarity with Certified Case Manager program requirements preferred.
• 2+ years in care coordination or ambulatory care preferred.

Responsibilities

• Outbound/inbound calls to coordinate patient care and appointments.
• Connect patients with resources and track adherence to care plans.
• Escalate clinical needs to RN Care Managers as needed.
• Assess home safety and social determinants of health barriers.
• Educate patients and promote self-management strategies.
• Document patient updates and progress.
