Job summary

Talentify is seeking a Care Coordinator to handle inbound and outbound calls, ensuring one-call resolution while adhering to Medicare and Medicaid guidelines. You will create referrals, investigate inquiries, and document details in our system with accuracy.

The role requires strong communication, empathy, and data-entry skills. This is a full-time remote position based in the United States, offering flexible schedules and opportunities to support patients and providers across the network.

Qualifications

• 1-3 years in related field preferred.
• Clear knowledge of Medicare, Medicaid & Commercial payer policies and guidelines for coverage, preferred
• Knowledge of Diagnostic Medical Expense and Medicare Administrative Contractor practices, preferred
• Clear understanding of Medical, Supplemental, and pharmacy insurance benefit practices, preferred
• Excellent written and oral communication, mediation, and problem-solving skills
• Strong people skills with flexibility, persistence, creativity, empathy and trust
• Robust computer literacy including data entry and MS Office-based software programs

Responsibilities

• Handle inbound and outbound calls to determine needs and provide one-call resolution
• Report adverse events within required timeframe
• Create and complete referrals and applications and update on policy changes
• Investigate and resolve patient/physician inquiries and concerns promptly
• Enter detailed information into proprietary software during calls
• Place outbound calls for patient follow ups or confirmations
• Provide superior customer support and empathy to patients
• Interact with patient referral sources to process new applicants
• Steward patient accounts from initial contact through final decision
• Prioritize multiple assignments with urgency
• Educate callers and provide effective processes and strategies
