Talkspace is seeking an experienced Care Coordinator Manager to lead the Referral Management team in New York. You will oversee a team of Care Coordinators who support patients from activation and appointment preparation through ongoing engagement, discharge, and transitions to higher levels of care.

You will collaborate with patients, providers, Talkspace facilities, and cross-functional partners to identify gaps and address barriers in care pathways, ensuring quality, timely access to services.
