You will collaborate with members and care teams to facilitate care, review records, and prepare care summaries to guide next steps and improve outcomes. The role requires strong organization, attention to detail, and the ability to work in EST/CST time zones, with travel occasionally for training or meetings.

Qualifications

• High School Diploma or GED required.
• Experience providing in-center or virtual support or interaction with patients.
• Experience in patient support, care coordination, or a related healthcare setting.
• Experience with data entry and maintaining information within an EMR system.
• Can work EST or CST.

Responsibilities

• Coordinate administrative, operational, and customer support activities for care management programs.
• Support assessment, coordination, and facilitation of members' care needs to improve health outcomes.
• Collaborate with members, providers, caregivers and care teams to support wellbeing and prepare care summaries for teams.
• Use discretion and judgment to prioritize requests and apply established procedures.
