The role requires 2+ years of related experience, strong MS Office skills, and excellent communication abilities. Location is remote with a 40-hour week and potential travel for training. Travel may be required occasionally.

Qualifications

• 2+ years of related experience.
• Proficiency with Outlook, Word, Excel, SharePoint and Teams.
• Ability to use electronic information applications/software programs.
• Exceptional verbal and written communication and interpersonal skills.
• Passion for contributing to better consumer experiences.

Responsibilities

• Support a member's optimal wellness state by coordinating services and resources.
• Identify and resolve barriers that hinder effective care.
• Monitor patient progress toward outcomes using assessments, data and conversations.
• Understand work area concepts, standards, regulations and operating procedures.
• Make decisions about work approach and priorities following established guidance.
