Job summary

Humana Inc. (Inclusa) seeks a Care Coach I to evaluate member needs and guide families toward resources for optimal wellness. Remote, 40 hours weekly, with possible travel for training or meetings.

You will coordinate services, monitor progress through assessments and active care planning, and ensure privacy and HIPAA compliance throughout care delivery. Required are 2+ years in health and human services, strong MS Word/Excel skills, and excellent verbal and written communication.

Qualifications

• Bachelor's degree in health and human services field.
• Knowledge of Microsoft Word and Excel.
• Exceptional verbal/written communication and interpersonal skills.
• Ability to use a variety of electronic information applications/software programs.

Responsibilities

• Evaluate member needs and requirements to guide toward resources for care and wellbeing.
• Coordinate services and monitor progress toward desired outcomes via assessments and care planning.
