Job summary

Humana seeks a Care Coach to assess members' needs and guide families toward resources to achieve optimal wellness. This remote role involves coordinating services, monitoring outcomes, and ensuring privacy and compliance with TB screening and driving requirements.

The position requires a health and human services degree, 2+ years' experience, and strong communication skills. A dedicated home workspace and vehicle liability insurance are required.

Qualifications

• Bachelor's degree in health and human services field.
• 2+ years of related experience.
• Knowledge of Microsoft Word and Excel.
• Exceptional verbal/written communication and interpersonal skills.
• Ability to use a variety of electronic information applications/software programs.
• Must be TB tested as part of Humana TB screening program.
• Must have a valid state driver''s license and maintain personal vehicle liability insurance.
• Must have a dedicated home office space with a locked door.

Responsibilities

• Evaluate members' needs and requirements to achieve and/or maintain optimal wellness.
• Guide members/families toward resources and facilitate interaction with services.
• Coordinate services and resources to support member care; monitor progress toward outcomes.
