Job summary

Humana Inc is seeking a Telephonic Care Coach to support patient wellness by coordinating services and resources, monitoring progress, and addressing barriers to care. The role reports to the Manager, Care Management and involves 40-hour weeks with potential weekend coverage and occasional travel to Humana offices.

Required: 2+ years of related experience, proficiency in Microsoft Office, strong communication skills, and a passion for improving consumer experiences.

Qualifications

• 2+ years of related experience.
• Proficiency with Microsoft Office Programs: Outlook, Word, Excel, SharePoint and Teams.
• Ability to use a variety of electronic information applications/software programs.
• Exceptional verbal/written communication and interpersonal skills.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

Responsibilities

• Employ a variety of strategies to support a member's optimal wellness by coordinating services and resources.
• Identify and resolve barriers hindering effective care.
• Monitor patient care to ensure progress towards outcomes using assessment, data, and conversations.
• Understand work area concepts, standards, regulations, and operating standards.
• Make decisions regarding work approach and priorities following documented procedures and regulations.
