Job summary

Humana Inc. is seeking a Telephonic Care Coach to support members’ wellness by coordinating services and resources. The role reports to the Manager, Care Management and requires 2+ years of experience, strong communication skills, and comfort with electronic health information systems.

The position is remote with occasional travel to Humana offices for training. Standard hours are 9am-5:30pm EST, 40 hours per week, with potential scheduling variations based on business needs.

Qualifications

• 2 or more years of related experience.
• Proficiency with Microsoft Office: Outlook, Word, Excel, SharePoint and Teams.
• Ability to use a variety of electronic information applications/software programs.
• Exceptional verbal/written communication and interpersonal skills.
• Passionate about contributing to an organization focused on improving consumer experiences.

Responsibilities

• Support a member's optimal wellness by coordinating services & resources.
• Identify and resolve barriers that hinder effective care.
• Ensure patient progresses toward outcomes by monitoring care via assessment, data and conversations.
• Understand work area standards, regulations and operating procedures.
• Make decisions regarding own work priorities following established guidance.
• Work is guided by precedent and documented procedures/regulations.
