Humana is seeking a Care Coach to evaluate member needs and guide families toward appropriate care resources. This remote role involves structured assessments, care planning, and coordinating services to improve wellness.

Training runs for the first 4 weeks, 8:30 AM–5:00 PM EST, followed by 10:00 AM–6:30 PM EST, Monday–Friday. Qualified candidates have an Associate degree, 2+ years in direct adult patient care, and strong communication skills.

Qualificações

• Associate degree required; 2+ years in direct adult patient care.
• Experience with healthcare team interactions.
• Strong computer proficiency and EMR navigation.

Responsabilidades

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical and community resources to support health outcomes.
• Identify and resolve barriers that impede care and independence.
• Monitor progress and adjust care strategies as needed.
• Maintain strict adherence to compliance in a regulated program.
• Learn and navigate multiple computer systems while engaging members.
