Job summary

Humana is seeking a Care Coach to evaluate members' needs and guide them toward optimal wellness, coordinating with clinical and community resources in a remote setting.

The role requires an associate degree and 2+ years in direct adult patient care, with strong EMR proficiency and excellent communication skills. Training is 4 weeks, followed by standard 10:00 am – 6:30 pm EST hours, Monday–Friday.

Qualifications

• Associate degree required.
• 2+ years of direct adult patient care experience.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn and navigate EMR systems.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical, community, and multidisciplinary resources; ensure compliance.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust care strategies.
• Maintain compliance within a regulated program.
• Navigate multiple computer systems while engaging members in a fast-paced environment.
