Job summary

Humana is seeking a Care Coach to evaluate members’ needs and guide them toward resources to achieve optimal wellness. The role is remote with training in EST hours, followed by Mon–Fri schedule and adherence to compliance requirements.

The ideal candidate holds an Associate degree with 2+ years of direct adult patient care, strong communication, and solid computer skills, including EMR familiarity. Occasional travel to Humana offices may be required.

Qualifications

• Associate degree required.
• Minimum 2 years of direct adult patient care experience (e.g., long-term care, home and community-based services, hospital/facility setting, case management, managed care).
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn and navigate electronic software programs, including electronic medical records.

Responsibilities

• Conduct telephonic outreach to members with multiple chronic conditions and barriers.
• Assess member needs to develop and manage individualized care plans.
• Coordinate services and connect members to clinical, community, and multidisciplinary resources.
• Identify and resolve barriers that impede effective care and member independence.
• Continuously monitor progress toward health outcomes and adjust care strategies.
