Job summary

Humana is seeking a Care Coach to evaluate members’ needs and guide them toward appropriate resources to support optimal wellness. The role is remote with scheduled training and a fixed first-month cadence.

The Care Coach will develop individualized care plans, coordinate with healthcare teams, and monitor progress while ensuring compliance in a highly regulated program. Training runs 8:30 AM–5:00 PM EST for four weeks, followed by 10:00 AM–6:30 PM EST, Monday–Friday.

Qualifications

• Associate degree in any course.
• Minimum 2 years of experience in direct adult patient care.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and proven ability to learn EMR systems.
• Exceptional communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical, community, and multidisciplinary resources.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust care strategies as needed.
• Maintain strict adherence to compliance within a regulated program.
• Navigate multiple computer systems while engaging members.
