Job summary

Humana is seeking a Care Coach to evaluate member needs and guide access to resources that support optimal wellness. You will develop individualized care plans, coordinate services, and help members overcome barriers in a collaborative, compliant environment.

This remote role requires strong communication, EMR proficiency, and the ability to work with diverse care teams. Training lasts four weeks, followed by a standard M-F schedule with structured hours and occasional travel for meetings.

Qualifications

• Associate degree in any course.
• Minimum 2 years in direct adult patient care or case management.
• Experience interacting with healthcare team members and navigating EMR systems.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess needs and develop individualized care plans.
• Coordinate services with clinical, community, and other resources.
• Identify and resolve barriers to care and member independence.
• Monitor progress and adjust strategies to achieve health outcomes.
• Maintain compliance within a regulated program.
