Humana is seeking a Care Coach for a remote role in the United States. You will evaluate member needs, guide families toward resources, and develop individualized care plans while navigating multiple computer systems in a fast-paced environment.

Training runs 8:30 AM–5:00 PM EST for four weeks, then 10:00 AM–6:30 PM EST, Monday–Friday. The role requires an Associate degree and at least 2 years in direct adult patient care, with strong communication and EMR skills.

Qualifications

• Associate degree (any field) required.
• Strong interpersonal/communication skills for internal and external customers.
• Proven ability to learn and navigate electronic medical records systems.

Responsabilités

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess needs and develop individualized care plans.
• Coordinate services with clinical and community resources to improve health outcomes.
