Job summary

Humana is seeking a Care Coach to remotely evaluate and support members toward optimal wellness. This role guides members and families toward appropriate resources and coordinates with clinical and community services to ensure effective care.

The position requires an Associate degree with at least 2 years in direct adult patient care, strong communication, and proficiency with EMR systems. Training in a remote setting precedes regular 10:00 AM–6:30 PM EST hours and occasional travel.

Qualifications

• Associate degree in any course.
• Minimum 2 years of experience in direct adult patient care.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn EMR systems.
• Exceptional communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical and community resources to support health outcomes.
• Identify and resolve barriers to care and member independence.
• Monitor progress and adjust care strategies as needed.
• Maintain adherence to compliance in a regulated program.
• Navigate multiple computer systems while engaging members.
