Job summary

Humana is seeking a remote Care Coach to evaluate members’ needs, develop individualized care plans, and coordinate services with clinical and community resources to support optimal health outcomes. The role requires an associate degree and at least two years of direct adult patient care experience, with strong communication and EMR proficiency.

Initial training runs 8:30 AM–5:00 PM EST for four weeks, followed by standard hours Monday–Friday, 10:00 AM–6:30 PM EST.

Qualifications

• Two years of direct adult patient care experience (e.g., long-term care, home health, hospital, case management).
• Experience interacting with healthcare team members.
• Strong computer proficiency and ability to learn and navigate electronic software quickly.
• Excellent communication and interpersonal skills to build rapport with customers and stakeholders.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess needs and develop individualized care plans.
• Coordinate services and connect members to clinical and community resources.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust care strategies as needed.
• Maintain compliance in a regulated program.
• Navigate multiple computer systems while engaging members.
