UnitedHealth Group is seeking Wellness Coordinators in Missouri to provide care coordination and administrative support for behavioral health and substance use needs. This role focuses on engaging members, screening gaps in care, and developing care plans to improve access to treatment.

Responsibilities include working with clinical teams, documenting in electronic systems, and coordinating resources. A 40-hour weekly schedule is typical, within 8:00 am–5:00 pm CST, with remote work from

Qualifications

• 2+ years of experience in behavioral health and case management
• 1+ years of computer proficiency experience
• 1+ years of Dual diagnosis experience with mental health and substance abuse
• 1+ years of experience handling sensitive issues with members and providers in a confidential manner
• Access to a designated quiet workspace in your home with the ability to secure PHI
• Live in a location that can receive UnitedHealth Group approved high-speed internet connection
• Ability to work a 40-hour work week in CST

Responsibilities

• Wellness Coordinators work in an inbound and outbound member queue
• Documentation in multiple electronic documentation systems
• Coordinate information to clinical team partners
• Provide behavioral health benefit education
• Contact members to screen for and identify gaps in care
• Provide network referrals and appropriate community resources to help the member engage with mental health and substance use treatment
• Administrative support
• Performs other duties as assigned
