Job summary

UnitedHealth Group is seeking a Wellness Coordinator to support members with behavioral health needs in a remote nationwide role. You will assist in screening for care gaps, developing care plans, and coordinating with clinical teams to connect members with needed resources.

The position involves schedule Monday–Friday, 40 hours weekly, with telecommuting options and comprehensive benefits. A strong emphasis on privacy, PHI protection, and patient-centered care is required.

Responsibilities

• Inbound and outbound member queue management
• Documentation in electronic systems
• Coordinate information to clinical team partners
• Provide behavioral health benefit education
• Screen for gaps in care with members
• Provide network referrals and community resources to support engagement with mental health and substance use treatment
• Administrative support
• Performs other duties as assigned
