PeopleOne Health is hiring a Behavioral Health Navigator to support mental, behavioral, and social needs within the community. You will work with clinicians, social workers, and external partners to coordinate therapy, psychiatry, and health resources.

The role emphasizes SDOH screenings, care coordination, and member education, serving as a liaison between care teams and community organizations. Remote work with regional coverage is possible.

Formación

• Bachelor’s degree or CMA with relevant experience in behavioral health or primary care.
• Experience in care coordination, referrals, or patient navigation is preferred.
• PHQ-2, GAD-7 or similar screenings experience is required or beneficial.

Responsabilidades

• Support social worker in addressing mental, behavioral, and social needs for the patient population within the area.
• Coordinate care for therapy, psychiatry, and mental health resources with external services.
• Provide SDOH screenings, resources, and referrals.
• Collaborate with a multidisciplinary team to align physical and behavioral health needs.
• Conduct community outreach and build partnerships with local organizations and support programs.
