Qualifications

• Must meet one of the following: 2-year degree or higher with 3+ years of clinical experience; LPN with 3+ years of clinical experience; 5+ years of relevant experience, including 3+ years of clinical experience.

Responsibilities

• Engage members face-to-face and/or telephonically to complete needs assessment across medical, behavioral, functional and social needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness and social determinants.
• Partner with care team, providers, and community resources to execute the care plan.
• Provide education and coaching to support member self-management and lifestyle changes.
• Coordinate discharge planning and transitions after ER, hospital, or SNF stay.
• Advocate for members to ensure their needs are represented by the care team.

Education
