UnitedHealthcare, a brand of UnitedHealth Group, seeks a Health Coach Field Care Coordinator to manage a panel of members with low-to-medium complexity medical and behavioral needs. You will coordinate medical, behavioral, and social needs to optimize care utilization and quality.

Location flexibility is available for Missouri residents with up to 25% travel. The role requires strong teamwork, care planning, and patient education to promote self-management and improved outcomes.

Qualifications

• Must meet one of the listed degree/experience combos.
• 1+ years of MS Office (Word, Excel, Outlook) experience.
• Driver's license and reliable transportation; able to travel within territory.
• Ability to travel about 25% of the time; MO resident.

Responsabilités

• Engage members face-to-face and/or telephonically for comprehensive needs assessments.
• Develop and implement person-centered care plans for chronic conditions and social determinants.
• Collaborate with care team, providers, and community resources to execute care plans.
• Provide education and coaching to support self-management and healthy lifestyle changes.
• Support discharge planning and coordinate care transitions after ER visits or SNF stays.
• Advocate for members to ensure their needs are represented by the care team.

2-year degree or higher with 3+ years of clinical experience

LPN with 3+ years of clinical experience

5+ years of relevant experience including 3+ years of clinical experience

CCM certification

Community case management experience
