Job summary

UnitedHealth Group is seeking Wellness Coordinators to provide care coordination and administrative support for behavioral health and substance use needs. The role focuses on screening for gaps in care, developing care plans, and engaging members in appropriate resources.

Responsibilities include handling referrals, chart summaries, and PHI-secure documentation while working a 40-hour week in CST from a Missouri home workspace. Strong communication and organizational skills are essential.

Qualifications

• 2+ years of experience in behavioral health and case management.
• 1+ years of computer proficiency experience.
• 1+ years of dual diagnosis experience with mental health and substance abuse.
• 1+ years of experience handling sensitive issues with members and providers in a confidential manner.
• Ability to secure PHI and operate in a home workspace.

Responsibilities

• Manage inbound and outbound member queues and coordinate with clinical teams.
• Provide behavioral health benefit education and screening for gaps in care.
• Coordinate referrals and community resources for treatment access.
• Perform administrative support and maintain documentation.
