Job summary

Optum is seeking a Wellness Coordinator to provide care coordination and administrative support for behavioral health and substance use needs. You will locate and engage members, screen for gaps in care, and develop individual care plans to improve access to care.

You will also assist the Behavioral Health Case Management dept with referrals, chart summaries, and patient demographics verification, working a 40-hour CST-aligned schedule in Missouri.

Qualifications

• 2+ years of experience in behavioral health and case management.
• 1+ years of computer proficiency experience.
• 1+ years of dual diagnosis experience with mental health and substance use.
• Ability to handle sensitive issues with members and providers confidentially.
• Access to a quiet home workspace with PHI security; reliable high-speed internet.
• Able to work a 40-hour CST-aligned schedule.

Responsibilities

• Wellness Coordinators manage inbound/outbound member queues.
• Document in multiple electronic systems and coordinate with clinical partners.
• Screen for and identify gaps in care; provide referrals and community resources.
• Provide administrative support to Behavioral Health Case Management.
