Qualifications

Responsibilities

• Conduct outreach to behavioral health patients who are lost to care or at risk of disengagement.
• Manage inbound and outbound telephone calls efficiently and professionally.
• Schedule, route, and manage patient appointments appropriately.
• Provide accurate information and facilitate access to services in a patient‑centered manner.
• Communicate effectively with diverse populations using English and Spanish.
• Maintain compliance with organizational policies, quality standards, and confidentiality requirements.
